Registration can be completed online prior to your first appointment. Our front desk will provide you with a user name and password.

Your first visit will consist of a consultation appointment explaining your diagnosis and treatment options.

In most cases, treatment can be started on the same day as the consultation.

Please provide the following information at the time of your consultation:

• Your referral slip 
• Form of payment, if applicable
• Insurance card/information, if applicable
